Providing the data is necessary - failure to provide it will result in the inability to identify, and thus enter the Company's premises.
– CLIENTS & CONTRACTORS: to conclude and perform the contract and to pursue any possible complaint claims:

Providing data is necessary for the conclusion and performance of the contract with REHAMED-CENTER Sp. z o.o. and implementation of the rights and obligations related to the complaint.

The data is processed on the basis of art. 6 sec. 1 letter F of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46 / EC, the so-called GDPR for the purpose of debt collection.

– CONTACT (e-registration): data necessary to answer questions on the contact form:
The data is processed on the basis of art. 6 sec. 1 letter f of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46 / EC, the so-called GDPR in order to establish and maintain contact, in particular when using the contact form.

Only authorized employees / associates of REHAMED-CENTER Sp. z o.o., have access to personal information, in particular employees of the department with which you are involved, people with access to monitoring recordings and entities providing services to REHAMED-CENTER Sp. z o.o. that require access to data, e.g. security, HR, IT company, etc.
You have the right to request from REHAMED-CENTER Sp. z o.o. access to your personal data and obtain a copy thereof, as well as requesting rectification, deletion of data, restriction of processing and the right to object to it being processed, the right to transfer data and the right not to be subject to automated decision making.

In the case of data processing pursuant to art. 6 sec. 1 lit. f of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46 / EC, the so-called GDPR, each person has the right to object to the processing of personal data at any time.
If the basis for the processing of personal data is the consent of the data subject, the consent may be withdrawn at any time without affecting the validity of the processing between the consent and its withdrawal.
Personal data will be processed - depending on the situation - until the consent is withdrawn, for the period resulting from the provisions of law (in particular labour law, tax and accounting regulations), for the period required for claims to be time-barred or up to 1 month in the case of monitoring recordings.
Personal data is not transferred outside the European Union or used in automatic profiling processes.
